Patient Guide

Appointment/Procedure Preparation

We offer access to care through a specialized appointment scheduling system called Advanced Access. Please call (937) 435-3546(937) 435-3546 to schedule an appointment.

We are committed to providing same-day care, as long as you call us early in the morning. Our phone lines open at 7:30 a.m.

What to Expect

If you are coming in for medication refills that require fasting labs, please schedule a lab appointment at least two days before your appointment. This ensures that we will have the results at your appointment. If you prefer, you can have blood drawn during your appointment.

Please bring your health insurance card and a list of your current medications, including vitamins and herbal supplements. If you are seeing other physicians (specialists, etc.), bring their names and addresses. Also, if you have experienced any medication and/or health changes, please inform our physicians.

If you have a co-pay, please be prepared to make payment at the time of service.

When you are low on medication or have no refills, please schedule an appointment with a physician. Some patients may need to schedule lab work.

After Your Visit or Procedure

If you have lab work or x-rays done during your visit, you will receive a card in the mail with the results and/or instructions from your doctor. If you do not receive results within a week of your visit, please call our office.

If we are scheduling an outside appointment, you should hear from our office and/or the specialist’s office within a week of your visit.  If not, please follow up with our office.
