We are currently accepting new patients. Please call our office at (513) 423-1399(513) 423-1399 Monday through Friday between 8 a.m. and 4 p.m. to schedule your appointment.

Be sure to give us at least 24-hour advanced notice if you need to cancel or reschedule your appointment. 

Appointment/Procedure Preparation

The length of your appointment depends on the reason for your visit.

Appointment types include:

  • Consult – allow 45 minutes
  • Follow-up visit – allow 15 to 30 minutes
  • Electromyography (EMG) – allow 45 minutes to 1½ hours. Download the EMG Instruction Sheet.

What to Expect

Please bring the following items to your first appointment:

  • All necessary forms
  • Picture identification card
  • Insurance card(s)

Please be sure to bring either your medications, or a list of the medications you are currently taking, along with your insurance card(s). All patients should come prepared to pay any co-pays, deductibles, or co-insurances at the time of their visit.

After Your Visit or Procedure

Your physician will discuss your specific care instructions with you during your appointment. You will also be provided all prescriptions needed until your next appointment.

Medical records are kept completely confidential and will not be released without your consent. View our privacy policy.

If you need a prescription refilled between visits, call your pharmacy and give your name, phone number, prescription number, medication name and dosage schedule. We are happy to refill prescriptions during business hours. Please allow three business days for refill requests to be called in to your pharmacy.

Billing and Insurance

Please verify your insurance coverage before your appointment. Find a list of the major insurance plans accepted by most Premier Physician Network here. Comprehensive Neurology Associates accepts most of these insurance plans.
