The Dulan and Moore Dulan Family Wellness Center is dedicated to caring for our patients with the same compassionate care with which we would care for our own families.

Appointment/Procedure Preparation

To schedule an appointment, call the office at (513) 932-7951(513) 932-7951. If you are already a patient with our office, you also can sign-up through MyChart – an Electronic Medical Record (EMR). In addition to scheduling appointments, MyChart allows you to review your medical record from home and communicate with the team at Dulan and Moore Dulan Family Medical Center.

Our office staff will provide patients with a same-day appointment whenever possible.

It is important to know what your insurance covers and how your insurance company prefers referrals, prescription refills, lab work and other processes to be handled. If you have any questions about your insurance coverage, contact your benefits administrator or insurance company.

What to Expect

Always bring your current driver’s license, insurance card and any co-pay required by your insurance.

Our office will provide you with information about patients’ rights under the Health Insurance Portability and Accountability Act (HIPPA), as required by federal law.

Patients scheduled for an annual physical are asked to fast for 12 hours before their appointment to prepare for possible blood work. Patients in need of medication refills are asked to bring a current list of all their medications, including all over-the-counter medications and vitamins.

Our office staff believes every patient is important, and we are dedicated to providing the best possible care. Sometimes, unexpected medical situations can cause patients to need extra time and attention. We appreciate your understanding in these cases. The same courtesy of extra time and attention will be given to you if you have additional medical needs.

The practice will make every effort to see patients who arrive late to appointments because of unforeseen events. Depending on the day’s schedule, however, it might be necessary to reschedule your visit to another day.

After Your Visit or Procedure

When your visit is done, you will be given an After Visit Summary, which includes the details of your visit, instructions, educational materials and suggestions. You also will be given the opportunity to sign up for MyChart – an Electronic Medical Record (EMR) – so you can review your medical record from home and communicate with the medical team through the secure Website. MyChart also allows you to schedule appointments, check your current and past lab results and ask other medical questions. Consider making your appointment for your follow-up visit before leaving your current appointment. You will have more options for a day and time that are most convenient to you, especially if you prefer late afternoon or early evening appointments. Same-day appointments are also available, and we will try to accommodate patients with those appointment times whenever we can. To make an appointment call the office at (513) 932-7951.

MyChart is the most efficient way to contact our office, though patients are also always welcome to call the office with questions. A team member will respond to all questions – whether asked by phone or through MyChart – in 24 to 48 hours. If you have a more immediate need to talk with your doctor, call the office to schedule a same-day appointment.

During appointments to manage prescriptions, your doctor will prescribe you with enough of your medication --including multiple pharmacy refills, if needed – until your next visit. If you are running low on your medication, first check with your pharmacy for existing refills. If there are no refills, schedule an appointment by phone or through MyChart because it is likely time for a follow-up appointment.

Billing and Insurance

Please verify your insurance coverage before your appointment. Find a list of the major insurance plans accepted by most Premier Physician Network online. Dr. Dulan and Dr. Moore Dulan accept most of these insurance plans. Once your insurance company is verified, our office will bill your insurance company for services rendered. You might be responsible for any portion of your bill not covered by your insurance company.

Other office visit charges – co-payments, deductibles and balances due – need to be paid in full at the time of service. We accept payment by cash, check or credit card (Visa, MasterCard, Discover and American Express).
