Appointment/Procedure Preparation

Greater Dayton Cardiothoracic and Vascular Surgeons is accepting new patients. Our patients are generally referred to us; however, you may contact our office by dialing (937) 208-7240(937) 208-7240.

We ask new patients to arrive 15 minutes prior to their initial appointment to complete any of the necessary forms from the list below: 

• Patient Registration Form - Adult
• Patient Registration Form - Minor
• Established Patient History Form
• Medical Release Form
• Verbal Communications Form
• Advance Directives Patient Education Form
• Advance Directives - Health Care Power of Attorney and Living Will

Due to the nature of our specialty, it may be necessary to change the date and time of your appointment should an emergency arise.

What to Expect

Please be prepared to present the following at the time of your appointment:

• Insurance card(s)
• Photo ID
• Complete list of medications, including dosage, name, and complete address of family physician
• New Patient Registration Packet (if mailed to home)

After Your Visit or Procedure

Following your visit with us, information will be provided to you regarding scheduling of tests and/or surgery as needed. We can go over any questions you may have at that time.
