Thank you for choosing Hand and Reconstructive Surgeons and Associates! We know you have choices when it comes to your care, and we appreciate that you’ve trusted your hands in ours.

Appointment/Procedure Preparation

Patients are seen by appointment only. Our friendly and courteous staff will do everything possible to make your visits as productive and pleasant as possible. However, because emergencies are not predictable, we may be delayed. If your doctor is running late, every effort will be made to let you know in advance.

We ask new patients to arrive 15 minutes prior to their initial appointment to complete any of the necessary forms from the list below: 

• Patient Registration Form - Adult
• Patient Registration Form - Minor
• Established Patient History Form
• Medical Release Form
• Verbal Communications Form
• Advance Directives Patient Education Form
• Advance Directives - Health Care Power of Attorney and Living Will

If you have had X-rays or testing related to the nature of your evaluation, please confirm that you are bringing them with you or that our office has received them.

Cancellations

We would appreciate knowing as soon as possible if you need to cancel or reschedule an appointment.

Please note: If your insurance requires that you have a referral authorization for your appointment with a specialist, you are responsible for verifying that it has been issued. The authorization MUST be in our office before you are seen. Failure to have this document will result in you being responsible for any charges incurred and denied due to lack of authorization.

What to Expect

Please be sure to bring either your medications or a list of the medications you are currently taking along with your insurance card(s). All patients should come prepared to pay any co-pays, deductibles, or co-insurances at the time of their visit.

After Your Visit or Procedure

Follow-up instructions will vary depending on what services are being performed. If a return appointment is needed, we are happy to schedule it at the end of your visit. We also place reminder calls 24 to 48 hours before scheduled appointments.

If a return appointment is not needed, we mail you a result card or call with your results within two weeks of the test.

Billing and Insurance

Please verify your insurance coverage before your appointment. Find a list of the major insurance plans accepted by most Premier Physician Network online. Hand and Reconstructive Surgeons and Associates accepts most of these insurance plans.

Workers’ Compensation
In order for you to receive full benefits, you must provide us with verification of any work-related injury or condition.
