Frequently Asked Questions

How do I schedule, change or cancel an appointment?

Please call our office at (513) 755-1912 to schedule an appointment. Please give at least a 24 hours notice if you need to change or cancel an appointment. You can also schedule or reschedule an appointment using the MyChart electronic medical record.

Are you accepting new patients?

Yes, we are accepting new patients of all ages.

Do I have to pay at the time of service?

Yes, all co-pays and previous balances are due at the time of service.

What form of payment do you accept?

We accept cash, checks, debit cards and credit cards (Visa, MasterCard, Discover and American Express cards).

What age does a patient have to be in order to be seen at Liberty Family Medicine?

We accept patients of all ages, from newborns to the elderly. All minors must be accompanied by a legal guardian.

What should I bring to my first appointment?

You should bring the following to your first appointment at Liberty Family Medicine: completed Patient Registration Form or Minor Registration Form current insurance card, photo ID, a list of your allergies, a list of the other physicians you see and the medical conditions you see them for, and all current medications (including any supplements and over the counter medications).

How early do I need to arrive for my appointment?

It is recommended that you arrive 10-15 minutes prior to your appointment to allow time to submit your paperwork and for the verification or update of your insurance and/or personal information.

Do you perform any lab tests at your facility?

Yes, we collect blood for lab tests to be sent to CompuNet Lab. We also perform some rapid in-house lab tests that provide immediate results. We do not perform any X-rays or other imaging tests at our facility.

How do I get a prescription refilled?

Our providers will make every effort to provide you with enough of your medication(s) to last until your next schedule appointment. So, it is likely that you are due to be seen if you are out of your medication(s). If you are running low on your medication and it is not yet time for you next appointment, please call your pharmacy and request a refill. In most cases, you will need to be seen for an appointment in order to get your prescriptions refilled. You will need to be seen for all new prescriptions.

What if I am sick and need to be seen immediately?

We do offer same-day appointments for sick visits.

What do I do if there is an emergency or if I get sick after normal business hours?

If there is a true medical emergency, then you are advised to call 911 immediately. If you become sick after normal business hours, and it is not a medical emergency, you may call our office at (937) 949-4464 and you will be connected to the on-call physician. Please be aware that the on-call physician cannot call in prescriptions or schedule appointments.
