Appointment/Procedure Preparation 

Any special instructions about your appointment will be discussed with you during the initial phone conversation.

What to Expect

New patients are asked to bring a list of their medications (including supplements and over-the-counter medications), a list of their allergies and insurance card(s) to your appointment.

Established patients should bring insurance card(s) to each visit. Also, each year you will be asked to complete an updated Patient Registration Form and to renew your permission to bill your insurance.

All patients should come prepared to pay any co-pays, deductibles, or co-insurances at the time of their visit.

After Your Visit or Procedure

At the end of your visit, you will be given an After Visit Summary, which includes your physician’s instructions and educational materials and suggestions. You also will be asked to sign up for MyChart – an electronic medical record – so we can communicate through our secured website. 

With MyChart, you can schedule appointment, check your current and past lab results, and ask other medical questions to the Middletown Medical Group team. Our office communicates lab and X-ray results promptly either by phone, letter or MyChart.

Billing and Insurance

Please verify your insurance coverage before your appointment. Find a list of the major insurance plans accepted by most Premier Physician Network here. Middletown Medical Group accepts most of these insurance plans.
