Appointment/Procedure Preparation

Welcome to Oakwood Primary Care. Please review important information about our practice regarding policy, procedures and more.

Please call (937) 208-7280 to schedule a new patient appointment. We are currently accepting new patients, and we strive to accommodate patients who require urgent care. Please arrive 15 minutes prior to your first office visit. For established patients, we offer “open access scheduling” via MyChart for your routine appointments. This allows you to schedule an appointment 24/7 from your computer or smartphone – no need to wait for the office to open!

Please be sure to notify us immediately of any changes in personal or insurance information.

You will be asked to update your health information when you are registering for preventative services.

What to Expect

All new patients need to complete a Patient Registration Form and Patient Information Form unless you are already in the system and have an updated form on file. A copy of this form as well as a Patient Information Sheet and Financial Policies form can be mailed or e-mailed prior to your appointment or can be downloaded online here.

Please bring your completed forms, picture ID, health insurance card(s), and a list of medications (including number of refills remaining and/or when you will need refills) and allergies to your appointment. We strive to provide our patients with enough refills until their next appointment whenever possible. We do not accept automatic refill requests from pharmacies (except mail order). If you need a refill, you may need an appointment. For your convenience, refills may be requested via MyChart.  If you have not yet signed up, please call our office when a refill is needed.

If your insurance requires a co-pay or if you have a deductible, please be prepared to make payment at the time of service. If, for some reason, you are unable to pay your co-pay at the time of service, a $10 fee will be added.

After Your Visit or Procedure

After your visit, you will be given instructions on follow-up appointments, tests, or consultations. Many test results are available via MyChart within a few days of testing.  If your test results indicate that you need treatment before your next appointment, our staff will contact you by phone or mail.  Please be aware that some testing processing takes longer than others.  Certain test results may take more than a week.  Abnormal test results will be discussed as soon as possible. If you do not receive results within 10 days of your visit, please call our office.
