Patient Guide

Ob/Gyn of Greene County looks forward to serving you as your partner in health care.

What to Expect

If you are a new patient, you will be asked to fill out a New Patient Registration Form. If you schedule your appointment more than a week in advance, we will mail you a patient packet, which includes a new patient questionnaire. You may also fill out this information upon arrival to your appointment. The New Patient Registration Form can also be downloaded if you would like to fill the forms out in advance. Please bring your insurance card and photo identification to your appointment. Any applicable co-pays are due at the time of service.

New Patients

New patients are asked to arrive 15 minutes before their scheduled appointment time to complete the registration process.

After Your Visit or Procedure

You will receive follow-up instructions at the time of your visit.

You also will be given the opportunity to sign up for MyChart – an electronic medical record which allows you to review your medical information online and communicate with the team at OB-Gyn of Greene County through the secure website. MyChart also allows you to schedule appointments, check your current and past lab results and ask other medical questions. 

Billing & Insurance

Hospital Pre-Registration

We encourage all mothers-to-be to pre-register for their birthing experiences. Taking the time to complete paperwork before you arrive to deliver your baby will make your check-in quicker, allowing us to have you in your room in less time. Pre-registration can be completed at your hospital's registration office or online.
