At Premier Cardiovascular Institute, we provide comprehensive, patient-centered care by building partnerships with our patients and their families. We welcome new patients. Please call the main number to the office location convenient to you to schedule your appointment.

Appointment/Procedure Preparation

Most new patients require a referral, so please make arrangements for the referral with your primary care physician a few days before your appointment.

Please make sure you bring:

  • Your photo ID
  • Your insurance card
  • All your current medications or a list of them

If you need to cancel your appointment, please do so 24 hours before your appointment time. We understand that your time is valuable and we’ll make our best effort to adhere to scheduled appointment times. However, emergencies may cause delays.

Medical records are completely confidential and will not be released without your consent. Please update us of any changes in address, telephone number or insurance coverage as they occur.

Please be prepared to pay any co-pays, deductibles, or co-insurances at the time of your visit. Patients who are unable to supply their insurance card(s) and/or payment will be asked to reschedule.

What to Expect

At your first appointment, you will meet with your physician and/or nurse practitioner for a comprehensive examination. Your current prescription medications will be reviewed at your appointment, and all necessary refills will be given to you.

After Your Visit or Procedure

Your physician will discuss your specific care instructions with you during your appointment. Follow-up instructions will vary depending on your needs and the service performed. If you need a return appointment, we will gladly schedule one at the end of your visit.

If you need any testing done, we will provide instructions to you and schedule your follow-up appointment. A provider will call you when your test results are available.

If you need a prescription refilled between visits, call your pharmacy and give your name, phone number, prescription number, medication name and dosage schedule. We are happy to refill prescriptions during business hours. Please allow 24 to 48 hours for refill requests to be processed.
