Patient Guide

Appointment/Procedure Preparation

To schedule an appointment, call our office at (513) 423-1211. New patients can download the following forms or request forms to be e-mailed or sent via U.S. Mail: New Patient Information sheet, a Patient Registration Form, and a Personal Health Risk Appraisal Form. 

Please complete these forms and bring them with you to your first appointment. You should also review the Notice of Privacy Practices prior to your first visit. 

Any special instructions regarding your appointment will be discussed with you during the initial phone conversation.

What to Expect

If you are a new patient, please be sure to bring your medications to your appointment as well as a list of allergies and your insurance card(s). Established patients should bring your insurance cards to each visit. You will be asked to complete a new Patient Registration Form annually so that your information is updated and you renew permission to bill your insurance.

All patients should come prepared to pay any co-pays, deductibles, or co-insurances at the time of their visit.

After Your Visit or Procedure

At the end of your visit you will receive an After Visit Summary which will include your physician's instructions and appropriate education materials or suggestions.  You will be asked to sign up for MyChart so that we will be able to communicate via our secured website, back and forth, as needed,   MyChart also allows you to schedule appointments, check your current and past lab results, and ask other medical questions.  Our office communicates lab and x-ray results promptly either by phone, letter, or MyChart.

For chronic illnesses, follow-up appointments will be scheduled every 3 months.

Electronic Medical Record (EMR)

During your visit, we enter important information, order tests, and list medications and other health information in your electronic medical record. This information is accessible to all Premier Health Partner physicians and hospitals to ensure you receive the highest level of care.

Information documented in your EMR includes:

  • Current medical conditions
  • Details of each visit 
  • Family health history
  • Immunizations
  • List of current medications
  • Personal health history
  • Preventive health screenings
  • Vital signs

How the Electronic Medical Record Benefits You

  • The EMR system ensures that your most accurate and up-to-date medical information is easily accessible should you visit a Premier Health Partner hospital or specialist. This is especially important if you have a medical emergency or illness that requires hospitalization.
  • We can get your test results quickly from services performed in a Premier Health Partner hospital or diagnostic facility, and laboratory services performed at Compunet Clinical Laboratories.
  • Referrals to other Premier Health Partner facilities are seamless. Instructions from your physician to the referred hospital or facility are sent to you electronically. The facility will contact you within 24 hours to schedule your services and instruct you on test preparation. Most test results are available for the physician to review electronically in 48 to 72 hours; many are available even sooner.
  • Prescriptions and refills of medications can be sent electronically to your pharmacy or to your mail order pharmacy service. This speeds the process for you, and improves communication and accuracy between the physician and pharmacist.
  • Your information is safe and secure, accessible only by those directly involved in your care. All healthcare service providers are bound by Premier Health Partner’s confidentiality and compliance policies, as well as government regulations, to protect your privacy when using the EMR.

Billing & Insurance

Please verify your insurance coverage before your appointment. Find a list of the major insurance plans accepted by most Premier Physician Network here. Premier Health Family and Geriatric Medicine accepts most of these insurance plans.
