Premier Urology Center

Patient Guide

Premier Urology Center is committed to serving men, women and children with their urology needs, including vasectomy, vasectomy reversal, prostate care, prostate cancer, urinary tract infections and more.

The office is conveniently located Middletown, which provides easy access for patients in both great Dayton and Cincinnati.

Appointment/Procedure Preparation

All new patients at the Premier Urology Center must be referred by their primary care physician. The referring physician’s staff can call Premier Urology Center at (513) 423-0739 to schedule an appointment while the patient is in their office or fax a referral to (513) 423-2265.

When a referral is faxed, the patient will receive a call from the Premier Center Urology within 24 hours or the patient can call the office to set up an appointment.

What to Expect

We ask new patients to arrive 15 minutes prior to their initial appointment to complete a Patient Registration Form and a Medical Information Form. These forms are also available to download online if you would like to fill them out in advance.

In addition, you may be asked to complete a Medical Records Release Form, which allows our office to obtain medical records from your previous doctors.

Please be sure to bring either your medications or a list of the medications – including dosage – you are currently taking, your insurance card(s), a photo ID and name and address of your family physician. All patients should come prepared to pay any co-pays, deductibles, or co-insurances at the time of their visit.

After Your Visit or Procedure

After each visit at the Premier Urology Center, patients will receive instructions for making a follow-up and/or for care after their specific procedure.

For example, a patient who just had a vasectomy should:

  • Take Tylenol to manage pain, as instructed by your doctor
  • Ice the groin area for four to five hours, as instructed by your doctor
  • Shower carefully after 24 hours; do not bathe for four to five days, as instructed by your doctor
  • Use gauze pads and antibiotic ointment for two days, as instructed by the doctor. Bring follow up semen samples, as instructed by your doctor

You also will be given the opportunity to sign up for MyChart – an electronic medical record which allows you to review your medical information online and communicate with the team at Premier Urology Center through the secure Website. Please verify your insurance coverage before your appointment. Find a list of the major insurance plans accepted by most Premier Physician Network here. Premier Urology Center accepts most of these insurance plans.
