Appointment/Procedure Preparation

To schedule an appointment, please call (937) 748-4211(937) 748-4211 or stop by our office. Please arrive early to all appointments to help us start your office visit at your scheduled time.

What to Expect

If you are a new patient, please be sure to bring your insurance card(s). Returning patients should bring their new insurance card(s) when applicable. Please bring your medication bottles or a current medication(s) list, including the medication name, strength, and number of times taken per day.

For routine follow-ups, please complete lab work one to two weeks prior to your appointment. This allows us to have your results available at your appointment time.

Please come prepared to pay any co-pays, deductibles, or co-insurances at the time of your visit.

After Your Visit or Procedure

After your visit, you will receive instructions for making a follow-up appointment as directed by your physician. If you had tests done, you will receive a phone call or a card in the mail with test results. If you have not heard from us within two weeks, please contact us.

If you need refills at your appointment, please see the nurse after you check in.
