Appointment/Procedure Preparation

We ask new patients to arrive 15 minutes prior to their initial appointment to complete any of the necessary forms from the list below: 

• Patient Registration Form - Adult
• Patient Registration Form - Minor
• Established Patient History Form
• Medical Release Form
• Verbal Communications Form
• Advance Directives Patient Education Form
• Advance Directives - Health Care Power of Attorney and Living Will

What to Expect

We may need to verify that the information we have is correct, so please have your identification and insurance card ready.

After Your Visit or Procedure

We strive to process laboratory test information efficiently, contacting you within three business days whenever possible. We make our best effort to provide same-day service to your pharmacy for prescriptions and refills. The turnaround time over holidays and weekends may be from 48 to 72 hours.

Billing and Insurance

Find a list of the major insurance plans accepted by most Premier Physician Network practices online. Studebaker Family Practice accepts most of these insurance plans.
