Appointment/Procedure Preparation

We are currently accepting new patients at Trotwood Physician Center. To schedule an appointment, please contact our office at (937) 208-7050(937) 208-7050.

Any special instructions will be given to you at the time you schedule your appointment.

We ask new patients to arrive 15 minutes prior to their initial appointment to complete any of the necessary forms from the list below: 

• Patient Registration Form - Adult
• Patient Registration Form - Minor
• Established Patient History Form
• Medical Release Form
• Verbal Communications Form
• Advance Directives Patient Education Form
• Advance Directives - Health Care Power of Attorney and Living Will

What to Expect

If you are a new patient, please be sure to bring your insurance card. We also ask that you bring your medication bottles instead of a list.

After Your Visit or Procedure

Your physician may request a follow-up visit. If you have blood work done during your visit with us, you will receive a results postcard in the mail within two weeks. We ask that you call the office if you do not hear from us in two weeks.
