Appointment/Procedure Preparation

We are currently accepting new patients at Fairborn Medical Center. Please call (937) 208-8155(937) 208-8155 for an appointment. Same-day appointments are also available.

We ask new patients to arrive 15 minutes prior to their initial appointment to complete any of the necessary forms from the list below: 

• Patient Registration Form - Adult
• Patient Registration Form - Minor
• Established Patient History Form
• Medical Release Form
• Verbal Communications Form
• Advance Directives Patient Education Form
• Advance Directives - Health Care Power of Attorney and Living Will

We will provide you with any special instructions, such as fasting for labs, when you schedule your appointment.

What to Expect

Please bring a health insurance card and a list of medications, including over-the-counter medications and vitamins, to your appointment.

If you have a co-pay, please be prepared to make payment at the time of service.

After Your Visit or Procedure

Immediately following your visit, we will provide instructions regarding any follow-up visits or referrals to outside providers. We will also provide patient education materials or written instructions pertaining to your visit before you leave the office.
