Appointment/Procedure Preparation

To schedule an appointment, please call our office at (937) 233-3910(937) 233-3910.

We ask new patients to arrive 15 minutes prior to their initial appointment to complete any of the necessary forms from the list below: 

• Patient Registration Form - Adult
• Patient Registration Form - Minor
• Established Patient History Form
• Medical Release Form
• Verbal Communications Form
• Advance Directives Patient Education Form
• Advance Directives - Health Care Power of Attorney and Living Will

What to Expect

If you are a new patient, please bring your health insurance card, a list of all medications, photo identification, and shot records, if available. Returning patients should bring their new insurance card(s) when applicable. You may be asked to complete a new Patient Registration Form once a year.

Please be prepared to pay for copays, deductibles, and coinsurances at the time of service.

After Your Visit or Procedure

After your visit, you will receive instructions for future appointments or referrals. We will call you or send you a letter for test results.
