Appointment/Procedure Preparation

To schedule an appointment, call our office at (513) 988-6369. Any special instructions for your appointment will be discussed with you during the initial phone conversation.

We ask new patients to arrive 15 minutes prior to their initial appointment to complete any of the necessary forms from the list below: 

• Patient Registration Form - Adult
• Patient Registration Form - Minor
• Established Patient History Form
• Medical Release Form
• Verbal Communications Form
• Advance Directives Patient Education Form
• Advance Directives - Health Care Power of Attorney and Living Will

Please review the Notice of Privacy Practices prior to your first visit.

What to Expect

New patients are asked to bring their current medications, including any supplements and over-the-counter medications, to their appointment, along with their insurance card(s) and a list of allergies.

Established patients also should bring their insurance card(s) to each visit. You will be asked to complete a new Patient Registration Form annually so that your information is updated and you renew permission for your insurance to be billed.

All patients should come prepared to pay any co-pays, deductibles or co-insurances at the time of their visit.

After Your Visit or Procedure

At the end of your visit, you will receive an After Visit Summary, which will include your physician’s instruction and appropriate education materials or suggestions. You will be asked to sign up for MyChart so that you can communicate with our office through our secured website, as needed. Through MyChart, you can schedule appointment, check your current and past lab results and ask medical questions.

Our office shares lab and X-ray results promptly by phone, letter or through MyChart.

Billing and Insurance

Please verify your insurance coverage before your appointment. Find a list of the major insurance plans accepted by most Premier Physician Network. Trenton Family Medicine accepts most of these insurance plans.
