How do I schedule, change or cancel an appointment?

Please call the office (513) 424-7291(513) 424-7291 to cancel any appointment, we require a 24-hour notice of any cancellations.

Are you accepting new patients?

Yes.

Do I have to pay at the time of service?

Yes, all co-pays and any outstanding balances are due at appointment.

What form of payment do you accept?

We accept cash, checks, and credit cards (Visa, MasterCard).

What age does a patient have to be in order to be seen at Middletown Family Practice?

We accept all ages, newborn to elderly.

What should I bring to my first appointment?

You should bring your insurance card, license/photo ID, current list of medications, and a list of allergies.

How early do I need to arrive for my appointment?

It is recommended that you arrive 10 minutes prior to your appointment.

Do you perform any lab tests at your facility?

Yes, we perform rapid flu, strep, UA, and glucose stick tests.

How do I get a prescription refilled?

Please call the office to request a refill or you can submit a request through MyChart.

Do you have acute appointments/same day?

Yes.

What do I do if there is an emergency or if I get sick after normal business hours?

If there is a true medical emergency, then you are advised to call 911 immediately. You may also call our office at (513) 424-7291(513) 424-7291 and it will connect you to the answering service, who will then notify the on-call physician.
