Appointment/Procedure Preparation

You should also review the Notice of Privacy Practices prior to your first visit.

Any special instructions regarding your appointment will be discussed with you during the initial phone conversation.

What to Expect

If you are a new patient, please be sure to bring your medications to your appointment as well as a list of allergies and your insurance card(s). Established patients should bring your insurance cards to each visit.

All patients should come prepared to pay any co-pays, deductibles, or co-insurances at the time of their visit.

After Your Visit or Procedure

At the end of your visit you will receive an After Visit Summary which will include your physician's instructions and appropriate education materials or suggestions. You will be asked to sign up for MyChart so that we will be able to communicate via our secured website, back and forth, as needed. MyChart also allows you to schedule appointments, check your current and past lab results, and ask other medical questions. Our office communicates lab and X-ray results promptly either by phone, letter, or MyChart.

For chronic illnesses, follow-up appointments will be scheduled every 3 months.

Billing and Insurance

Please verify your insurance coverage before your appointment. Find a list of the major insurance plans accepted by most Premier Physician Network here. Middletown Family Practice accepts most of these insurance plans.
